Prior To Your Visit
Before you come in we will need you to answer the following screening questions:
Have you had a fever in the past 14 days? Chills/Sweating?
Have you had a cough in the past 14 days?
Have you been fatigued or lethargic in the past 14 days?
Have you had a loss of taste or smell recently?
Have you had shortness of breath before in the past 14 days?
Have you cared for or been exposed to someone who is sick in the past 3 weeks?
If you have answered YES to any of the above, please contact our office. We may need to reschedule your appointment for a later date.
Patients must wear a face mask/covering for the entire time of their acupuncture visit. If you do not have a face covering, please let us know and we will do our best to supply you with one. Patients must wash or sanitize hands upon arrival. Use of waiting rooms will be limited and social distancing requirements must be met.
